2026 Clinic Registration Form

You must submit a new form for each clinic participant. Fill out this form for EACH player you are registering. You will get a confirmation to your e-mail address (if typed correctly) once you have successfully submitted the form.

Note: Payment must be received before your child can participate in the clinic.
